Guide to the APNIC ISP Request Form (text version)

Before you start

This guide will lead you through the steps required to request an IPv4 address allocation from APNIC. If you are not already a member of APNIC, please contact member-apply@apnic.net before completing the form.

Use this guide if you are completing the text version of the APNIC IPv4 ISP Request Form.

The IPv4 ISP Request Form is also available as an online form.

How to complete the APNIC ISP Request Form (text version)

The text form is intended to be completed in text and submitted by email to APNIC. Please note that it will first be parsed by machine, before it is passed on to the APNIC hostmasters. Therefore, you must ensure that:

  • your email is sent in plain ASCII text only
  • you do not modify any of the template lines starting with #[
  • you do not modify any of the field names in the templates.
