[GLOBAL-V6] Final version of IPv6 policy document

I've revised the policy document in response to comments that were
raised at the RIPE meeting and afterwards. All of these changes are
editorial (e.g., to remove internal inconsitencies, to use the terms
"allocate" and "assign" consistently, etc.).

This document is pretty much final at this point. It contains what was
agreed to at the RIR meetings. So at this point, I'll consider any
last-minute wording issues, but issues about substance will be taken
as input to a followup to the agreed-upon policy.
